Why are Wisdom Teeth Removed?

While most adult mouths have enough space in their jaws to accommodate 32 teeth, those who don’t often have to get some or all of their 4 wisdom teeth removed when they become “impacted.”

A normal human adult has a total of 4 wisdom teeth, 2 in the top row and 2 in the bottom row. These wisdom teeth can become impacted when there’s not enough space for them to erupt properly.

As a result, they get stuck in the jaw bone without fully emerging and can cause swelling, pain, discomfort, and even infection in the surrounding teeth. The best thing to do then is to extract these teeth before they cause any more damage.

If you are experiencing tooth pain, or pain in a specific section of your mouth please don’t hesitate to contact Smile Lab in Union Square for a prompt wisdom tooth removal. Remember, the earlier you opt for a removal, the less pain you have to suffer.

Smile Lab's Wisdom Tooth Removal Process

At Smile Lab NY here in Union Square, Manhattan, we take every precaution necessary to ensure your wisdom tooth removal is 100% safe, pain-free, and effective. We also do our utmost to be as transparent as we can with our patients during every consultation.

In fact, in the spirit of transparency, here’s Dr. Marcello Guglielmi’s precise wisdom teeth removal process.

Initial Evaluation and Examination

Before heading into surgery, Dr. Marcello will conduct a thorough examination of your mouth to pinpoint the location of your wisdom teeth. This may occasionally involve an X-ray.

Depending on how deep your wisdom teeth are lodged into your jawbone, Dr. Marcello may just apply local anesthesia to the affected area or sedate you fully with laughing gas. In case he chooses the sedation option a needle will be inserted into a vein in your arm for administration of the sedative.
There may be some gum tissue or bone impeding the removal of the wisdom tooth. In this case, the dentist would have to remove a portion of it to ensure a safe and unimpeded extraction.

The dentist will use specialized surgical instruments to slowly loosen and ease your tooth out of the bone.

Immediately after the procedure, the patient may be given gauze to bite down to help in developing a blood clot. Next, the patient will be put into the recovery room until the sedatives wear off and the breathing rate becomes normal.

The Longer You Wait, the Worse It Gets.

Impacted wisdom teeth don’t resolve on their own — they cause more damage, more pain, and more expensive problems the longer they’re left untreated. The good news? Dr. Marcello can have you evaluated and on a clear path to relief this week.

Signs You May Need Your Wisdom Teeth Removed

Wisdom teeth rarely announce themselves politely. Here’s what to watch for.

Persistent Pain or Pressure at the Back of Your Mouth

A dull, throbbing ache or sharp pressure at the very back of your jaw is one of the most common and telling signs that your wisdom teeth are impacted and pushing against surrounding teeth. Many patients initially mistake it for a regular toothache — but if the pain is concentrated toward the back of your mouth and keeps coming back, your wisdom teeth are almost certainly the cause. Don’t wait for it to get worse.

When a wisdom tooth is trying to push through the gumline but doesn’t have enough room to emerge properly, the surrounding gum tissue becomes inflamed, tender, and prone to bleeding. You may also notice the gum tissue behind your last molar looks raised or puffy. This swelling is a sign that the tooth is impacted and needs to be addressed before it leads to infection.
Impacted wisdom teeth can create significant pressure and inflammation in the jaw joint, making it uncomfortable — or even painful — to open your mouth fully. If you’ve noticed your jaw feels stiff in the morning or it’s become harder to chew without discomfort, impacted wisdom teeth may be contributing to the problem and should be evaluated by Dr. Marcello as soon as possible.

As impacted wisdom teeth push against neighboring teeth, they can cause your other teeth to shift and crowd — undoing years of orthodontic work and changing the way your bite feels. If your teeth seem to be moving, your bite feels different than it used to, or your teeth look more crowded than before, impacted wisdom teeth may be the hidden cause. An X-ray at Smile Lab Dentistry will confirm it quickly.

Why Trust Smile Lab With Your Wisdom Teeth Removal in Manhattan

Wisdom teeth usually develop and emerge in the 17-21 age range. This is also the best age to get the impacted wisdom teeth removed before they cause any permanent damage to the surrounding teeth.

That said, wisdom teeth removal is a delicate procedure that requires an expert’s hand to ensure that it doesn’t end up causing more pain and discomfort than before. Also, to ensure that it doesn’t end up causing irreversible damage to your gums or jaw bone, resulting in the need for an even pricier surgical procedure.

Dr. Marcello Guglielmi here at Smile Lab in Union Square New York has been a practicing oral and maxillofacial surgeon for over a decade. He’s also a board-certified oral and maxillofacial surgeon who received his certification from the American Board of Oral and Maxillofacial Surgery.

But above all that, Dr. Marcello Guglielmi has conducted hundreds of successful wisdom teeth removals for satisfied patients who remain connected with us here at Smile Lab Manhattan.

I've seen lots of different dentists over the years as my insurance has changed or I've moved. Smile Lab is the first place where I feel like they really care about me. I had some rough cavities filled and was nervous because of a previous bad experience. The dentist here had no problem giving me breaks or extra medicine when I needed it. He even gave my jaw a massage when I could close my mouth again! I'll definitely trust Smile Lab with my future dental care.

Office is in an awesome location. Right off the F train on freaking Broadway! You enter and are greeted professionally, the staff are all really nice and the whole office is squeaky clean. Dr. Waise has great bedside manner, easy going but very knowledgeable. Ive been having headaches for 8 years and within 5 minutes of meeting me he explained to me the neck and jaw issues that come from posture, recommended a solution and even loosened up some of the sore muscles in my jaw! Cleaning was great, everything was transparent and nobody hassled me for anything. Ill be coming back, for a filling, a cleaning even if its only for a jaw massage!

Questions About TMJ Treatment in Manhattan

Do I actually need my wisdom teeth removed?
Not everyone needs their wisdom teeth removed — but most people do. If your wisdom teeth are impacted, growing at an angle, causing crowding, or creating recurring pain, swelling, or infection, removal is almost always the right call. The best way to know for certain is a quick evaluation with Dr. Marcello, who will take an X-ray and give you an honest, straightforward answer about whether removal is necessary in your specific case — no pressure, no upselling.

Wisdom teeth — especially impacted ones — should be removed by an oral and maxillofacial surgeon, not a general dentist. Oral surgeons have advanced surgical training specifically for procedures involving the teeth, jaw, and surrounding bone and tissue. At Smile Lab Dentistry, all wisdom tooth removals are performed by Dr. Marcello Guglielmi, a board-certified oral and maxillofacial surgeon certified by the American Board of Oral and Maxillofacial Surgery — giving you surgical expertise and peace of mind in the same appointment.

The cost of wisdom tooth removal varies depending on how many teeth need to be extracted, how deeply impacted they are, and what type of anesthesia is used. Online estimates are rarely accurate for your specific situation. The best way to get a real number is to come in. Smile Lab Dentistry offers a free consultation with Dr. Marcello — he’ll examine your teeth, review your X-rays, and give you a clear, honest cost breakdown with no obligation. Most major insurance plans are accepted and flexible payment options are available. Call 332-237-6453 to book yours today.

Most patients feel back to normal within 3 to 5 days after a straightforward wisdom tooth removal. More complex extractions involving deeply impacted teeth may require up to a week of recovery. Swelling and mild soreness are normal in the first few days and are easily managed with over-the-counter pain relief and ice packs. Dr. Marcello will provide you with detailed post-operative care instructions to make your recovery as smooth and comfortable as possible — and the Smile Lab team is always available if any questions come up along the way.

Swelling is a completely normal part of the healing process after wisdom tooth removal. It typically peaks around 48 to 72 hours after the procedure and then gradually subsides over the following 3 to 5 days. Applying a cold compress to the outside of your cheek in the first 24 hours helps significantly. If swelling is worsening after the third day rather than improving — or is accompanied by fever or increasing pain — contact Smile Lab Dentistry at 332-237-6453 right away, as this can occasionally signal an infection that needs prompt attention.
Dentistry in Union Square is one of Manhattan’s top-rated destinations for wisdom tooth removal. Dr. Marcello Guglielmi — a board-certified oral and maxillofacial surgeon with over a decade of experience — has performed hundreds of successful extractions for patients across New York City. With state-of-the-art surgical technology, a genuinely comfort-first approach, and over 1,400 five-star reviews, Smile Lab Dentistry is the team Manhattan trusts when it comes to oral surgery. Book your free consultation today at 332-237-6453.
Not right away. For the first 24 to 48 hours after your extraction, hot beverages like coffee should be avoided. The heat can dissolve the blood clot forming in the extraction site — a clot that is essential for proper healing — and increase the risk of a painful condition called dry socket. Once the initial healing period has passed and Dr. Marcello gives you the go-ahead, you can gradually reintroduce coffee. When you do, skip the straw — the suction can also dislodge the clot and slow your recovery significantly.
